Iatrogenic Facial Paraliysis in Middle Ear Operations

Facial paralysis as a result of an operation is a dramatic situation facing the patient and the surgeon. In this retrospective review, iatrogenic facial paralysis during ear surgery is discussed. Between 1964-1992, 41 iatrogenic facial paralysis cases which happened during otologic surgery in the Department of Otolaryngology Head and Neck Surgery of Hacettepe University together with referred cases were presented. The most common operations causing paralysis was radical mastoidectomy and tympanoplasty. The most frequent site of injury was the tympanic segment. In case of minimal injury and nerve sheath trauma only decompression of the nerve was done. If the nerve lost its continuity a nerve graft or end-to-end anastomosis was performed. These findings were discussed with literature paying special attention to the points in order to prevent the injury of the nerve.
